Parent Checkout Form - BOA Chattanooga Competition

This form must be completed by parents/guardians who intend to check out their marching band student after the band is officially dismissed at the BOA Chattanooga competition.

Students who are checked out will not return to Lambert with the band buses, chaperones, and directors.

- Complete the official checkout routine before leaving with my student.
My student will not travel back to Lambert with the marching band group, directors, or chaperones.
